Before Your Visit
Filling out your paperwork ahead of time means more of your first visit is spent talking with Dr. Park, and less of it spent at the front desk. Our secure online forms take just a few minutes.
If you are coming in for an airway or sleep concern, we may also ask you to complete a short sleep questionnaire so we can make the most of your time together.

Insurance
If you have dental insurance, we are happy to help you understand your coverage and file your claims for you. Our team will work to help you get the most from the benefits you already have.

Financing & Membership
Good dental care should fit your life and your budget. We accept cash, checks, and all major credit cards, and we offer flexible financing through CareCredit, Cherry, and in-house payment plans.
No insurance? No problem. Our Green Dental Wellness Plan is a simple annual membership for patients without insurance. It includes your routine exams, cleanings, and necessary x-rays, plus savings on other treatments, with no yearly maximums, waiting periods, deductibles, or claim forms.

A little transparency up front, so there are no surprises.
Before treatment, we provide a clear estimate of what is recommended and what your responsibility will be, honored for up to six months.
Payment is due at the time of your appointment. We accept cash, checks, and major credit cards, with financing available.
We reserve unhurried time just for you, so we kindly ask for 48 hours notice if you need to reschedule.
